Congestive Heart Failure ProgramCaring for people with heart disease in their homes or wherever they reside. Pulmonary Resource ProgramProviding education and support to patients with lung disease and their families. Pediatric ServicesProviding specialized services for children with life-threatening conditions while supporting their families. Health Care DecisionsEducating and helping people complete advance directives stating their wishes for end-of-life care. BereavementSupport to individuals and families for 13 months following a death. 11th Hour ProgramProviding volunteers and staff members to support families or patients who are alone and nearing death.
