A Teen Volunteer Coordinator will contact you for a short interview. Bring the following completed forms to your interview:

Teacher/Counselor Reference Form


Teens in Nursing Volunteer Orientation

Nine hours of orientation help prepare you to meet the needs of patients and their families. Subjects include history and the concept of hospice care, ethics, communication, death and dying, and grief and loss. To register for orientation, please submit your Teens in Nursing application first, then contact Lori Motola, Teens in Nursing Volunteer Coordinator, at 602.636.6342 or lmotola@hov.org.

Orientation takes place at Phoenix College, 1202 W. Thomas Rd., Phoenix, AZ 85013. The next orientation is scheduled for October 2011.
