Notice to Patients
It was learned on January 23, 2023 that one email account experienced unauthorized access affecting a small subset of patients referred to or receiving services from Hospice of the Valley predominantly between January 2019 and October 20, 2022. No information maintained in the medical records system was accessed and no medical claim, payment card or social security numbers were exposed. Information accessed may include: patient’s name and/or medical record number, dates, phone number, address, health insurance policy identifier, diagnosis or treatment. There is no evidence of misuse of information, however out of an abundance of caution, we are providing notice to those who may have been affected, including steps to protect personal information. Monitoring for identity theft and fraud is recommended by reviewing credit reports, account statements and explanation of benefit statements to guard against unauthorized transactions or activity. We reviewed security policies and engaged a national security firm which confirmed that effective security measures are in place.
